It is linking to entry_id 84 which doesn’t exist anymore (deleted via EE). I tried updating the module and still getting the error message. Any thoughts?

Kevin, I patched the file on your server. Not sure why, but the member data was not associated with the approval, which I haven’t seen happen before.
